Please make an appointment to visit your Primary CareVA physician.--During your appointment - request acupuncture treatments and ask your physician to write a referralfor Amazing Acupuncture.It may take a few weeks to a month for our clinic, and for you, to receive your authorization. When we get it - you can set your first appointment and will receive unlimited acupuncture treatments, free of charge, as a veteran benefit.
Now accepting Medicare Advantage Plan Insurance
Please provide us with copies of the front and back of your Medicare Advantage card and your Date of Birth. Our front desk staff will call to verify your coverage. If so, you will receive 20 acupuncture sessions a year, with a small co-pay.
Workers Compensation Claims
Amazing Acupuncture is an approved provider for the State of Arizona Workers Compensation Association. If you or someone you know has suffered an injury on the job - please call us right away to have acupuncture benefits added to your bodily injury claim.
Accident Injury Acupuncture
If you or someone you know has been in any kind of vehicular accident within the last 2 years as a pedestrian, driver or passenger - please call us. We will do our best to get acupuncture benefits added to your bodily injury claim.
